Tharaka Nithi County has promoted 21 medical consultants after signing a deal with the Kenya Medical Practitioners Pharmacists and Dentists Union (KMPDU).
The promotions, which bypassed job adverts and interviews, come under a Return-to-Work Formula agreed on May 8, following talks with the Council of Governors in April.
Eleven doctors have received promotion letters this month, with the remaining 10 expected to get theirs in June.
KMPDU Secretary General Dr Davji Atellah said the promotions were long overdue and urged other counties to act.
“Doctors have waited long enough. Fair promotion is not a favour, it’s a right,” he said.
Tharaka Nithi becomes the latest county to implement such reforms, after similar moves by Nandi and Nyeri in 2023.
The union says delays in upgrading senior doctors have weakened public hospitals and stalled career growth.

Attellah said the union expects counties to fully implement the Return to Work Formula, which was designed to resolve long-standing pay and promotion grievances among public sector doctors.
